          JOB SUMMARY: In collaboration with the Clinical Resource Coordinator and the Continuing Care Manager, address social and emotional adjustments and complications of illness resulting in barriers to treatment, risk to patient health, safety, and obstacles to discharge or continuing care. Provide assessment, counseling, information and referral, advocacy services, and other resource assistance to both inpatients and outpatients either directly or through consultation.

          MINIMUM EDUCATION & EXPERIENCE: MSW from an accredited school of social work with a minimum of 2 years of post graduate practice in a health care setting required. May require an LICSW in the state of NH. Excellent interpersonal and communication skills required. Demonstrated ability to meet the Clinical Social Worker level I criteria.
